I have to take out your #1. It was tragic and horrific, but I'm not sure controversial is the best fit.

Lynching The Bossman

The Undertaker is fighting Big Bossman in a Hell in a Cell match. Taker ends up putting a noose around Bossman and lynches the guy from the top of the cage.

Yes it is fake, but children are at this event and they hang a guy?

Putting a noose around a mans neck and hanging him from the rafters is NOT alright for live audiences that contain children.

I'll give my 4th next as well.

Those plus the TNA PPV where Hardy got high b/w the time he showed and it was time for the main event and Sting had to grab him and just pin him in like a minute.

Also Superstars of Wrestling. This featured Lou Albano mumbling incorhearently on commentary and a drunk Jake the Snake Roberts in the main event. When Jake came out to wrestle he used Damion as a substitute willy for all the crowd to see. At that point Capt Lou made the mainevent into a tag team to keep Roberts out of the ring.
